Withdrawal Form Request a withdrawal from your account – either full or partial*
Switch Form Request switches between investment options*
Transfer Authority Form Request monies held with external funds be rolled over to IRIS.
Change of Income Payment Amount and Frequency Form Change income payment amount or frequency*
Change of Member Details Form  Change of address and bank account details.
Binding Death Benefit Nomination Form Make a binding nomination, whereby the Trustee is required to pay your benefit to the dependant(s) you nominate in the event of your death.
Death Benefit Claim Form Application to claim a death benefit as beneficiary of a deceased IRIS member.

*Processing your request

If your completed request is received in the IRIS Administration office before 4pm (AEST) on a Wednesday, it will generally be processed effective that Wednesday. If your request is received after 4pm, your request will generally be processed effective the following week. You will generally be paid seven working days after we have processed your request.

Processing all transactions and buying and selling assets only once a week enables IRIS to keep the costs down.

After processing your request(s) we’ll send you a letter confirming the details of your withdrawal, switch or change of income payment.

Fax requests for amounts up to $50,000 will be accepted, however the original signed form must also be mailed to us.
